Chain Of Custody

The new version five of the ACZ Chain of Custody is available for download. You may enter information directly into the new Chain of Custody form using Adobe Acrobat Reader and print out what you have entered. If you have Adobe Acrobat (full version), you will be able to save the data you have entered. Some Project Managers feel that entering the data in the on-line form with contact and sample information, printing the form, and then leaving the samplers to fill in the sample collection date and time increases the accuracy of the Chain of Custody. Feel free to download this file to your network/PC. You can always download and fill out by hand if you prefer. Check back here every once in a while as there is another Chain of Custody revision due out soon.
